Yorkshire and Humber Care Record

Adult Social Care Services are one of the partner organisations in the Yorkshire and Humber Care Record (YHCR).

The YHCR allows health and social care professionals to see information from many organisations about a person that they are supporting. This is to help close gaps in health data. So that people have a joined-up experience when accessing health and care services. This also helps to ensure patient safety and informed decision-making.

The types of data shared by Mid-Yorkshire includes:

patient demographics
inpatient appointments
emergency department visits
documents including discharge summaries, clinical letters.
